Group Education in Diabetes

We designed a Learner Centred group education model for people living with Diabetes based on principles of COM-B. People were empowered through peer learning and creating space so all voices could be heard. In collaboration with a primary care family health team, we designed a learned centred model for group education, that started with a BRIGHTSPOT peer using an interview style ‘talk show’ method so participants could hear first the experiences of someone like them. Participants were then engaged in discussion with the BRIGHTSPOT to help reduce fears, accelerate intrinsic motivation and inspire possibilities. This method helps to move people from per-contemplative to readiness as it is focused on them. The design of the learning session was dynamic exchanges between participants and the team, each focusing on the area of their expertise ie: dietitian, CDE, family doctor, ensuring that the learning points were connected to the people and what they are trying to accomplish and not ‘top down’. The learning session was two part and participants had activities to do in between the sessions and they were able to share back their stories with the health care team. Impact included improved self management and engagement in care from participants along with appreciation for the model of inclusivity of their lived experiences as an important part of the learning. The model was part of a poster accepted and presented at AFHTO.
